SOP FOR MAJOR ACCIDENT/INCIDENT REPORTING
PURPOSE : An accident always results in diversion of resources from any normal business activity. In certain cases, publicity generated from an accident can result in a damaging effect on the image of the company. Thus it is imperative that top management is informed immediately about such accidents so that appropriate measures can be taken to minimize any adverse business impact.
SCOPE : Accidents of the following types (given below) occurring within the plant premises have to be informed at once.
Fire or explosion resulting in loss of life and / or property
Accident resulting in loss of life or major injury requiring hospitalization.
Major spillage of chemicals or gas leak that would endanger the environment or community surrounding the plant.
Any other type of accident / incident should be informed to corporate EOHS along with the monthly / Annual report.
RESPONSIBILITY : Plant Head
ACCOUNTABILITY : Location Head
PROCEDURE :
The Plant Head/location Head or his designee will immediately after the occurrence of an accident inform his immediate superior about the accident.
Information to be given will include brief details as to what had happened and the nature of injuries received.
A copy of this message will be sent on the mail to PLANT HEAD.
The causes of the accident will be investigated by a team comprising of :
- a representative from production
- a representative from engineering
- a representative from stores / personnel
- the Location EOHS manager and
- any other person decided by Location Head
In case of a major accident, the Corporate EOHS will carry out the investigation if so desired by the top management.
The corrective and preventive actions recommended by the investigation team can be discussed with Corporate EOHS if so desired by the Location Head.
A copy of the investigation report will be sent to all the safety committee members.
The Location Head will nominate a responsible manager to ensure that all the accepted recommendations have been complied with.
VERIFICATION :
Corporate EHS will ensure the implementation of this procedure.
